Referral Process

STEP 1: Consultation

We begin with a phone consultation to help determine your goals, preferences and needs in order to recommend the most suitable environment for your self or family.

STEP 2: Follow Up on Your Request

Then we talk by phone or in person to evaluation and determine lifestyle choices, health, budget and desired living location. Whenever possible we personally meet with the client to fully understand how we can recommend the environment that will meet and exceed our client’s expectations.

STEP 3: Tours of Potential Options

We pre-screen communities, matching the client’s care needs and preferences with community services. Thorough evaluations allow us to recommend specific communities for tours, which include visiting with residents and dining in each community. We are happy to accompany you during the initial introductions on tours. We will make sure that all of tour questions are answered and that your and your loved one is comfortable throughout the entire process.
Follow-up tours to a smaller group of selected communities encourage clients to explore the services of each potential community. At this time we ask our clients to revisit specific aspects of their choice in each community. We will assist you in making this important decision. We will review all of the options with you and assist in choosing the one that is best for your loved one. Whether this is an urgent decision or you need to prepare for the future, the final decision regarding time-frame for placement, and community choice belongs to the clients.

STEP 4: Personal Concern & Follow Up

We are concerned and caring, attending to the desires and needs of clients and their families. Once you have chosen an assisted living setting we offer follow-up services with both the client and family. The purpose of our home visits is ensuring a successful transition; where the senior will be comfortable, welcomed and cared for in their new environment.
